
Youth Community Case Management coordinates services for youth up to the age of 20. Qualifying diagnoses may include a combination of physical, medical, mental health, and developmental disabilities. It’s imperative to understand each family’s unique dynamic to optimize the success of their child. Providing opportunity and growth for the child but seeing success for the entire family is the achievement that the team supports. The Wraparound process is used to engage, prepare, and implement phases of achievement. A Strengths Inventory is created that include nine components of a youth's environment: Family, Living Situation, Medical, Well Being, Social/Recreational, Legal, Safety, Educational/Vocational, and Family Values
